Follow-up to #255: the folder goes back under All Subscriptions, above the pinned feeds, and New signal becomes a + at the right of the folder's own row instead of a row inside it.

Done in 8d93b58, deployed to ipx-web: the Signals folder is first in the feed list, right under All Subscriptions, and New signal is a + at the right of its row. Making a Signal opens the folder to show it.
